Application Packet

The application packet must include:

• A completed dietetics major application form (below)
• Official transcripts from all colleges and universities other than James Madison University.
• Unofficial JMU transcript.

Complete this DPD application by February 15 of your sophomore year. You must complete all the prerequisites to the program prior to Fall of your Junior year.
